Letter of Inquiry: Suggested Outline
A suggested outline for the Letter of Inquiry is as follows (note that budget is in addition to the 3 pages):
a) Introduction - A brief statement of the amount you are requesting and the project you are requesting it for.
b) Background - A brief background statement about your organization.
c) The Need - Describe the problem or challenge you are trying to solve.
d) The Program - Describe the goals you hope to achieve with the project. What are the methodologies or activities you will put in place to meet these goals? What are the numbers to be served? Who are the key people involved? What is the projected timetable and anticipated completion date?
e) Outcome - Describe how you will know you have been a success. What will you see different and better as a result of your work? Describe any prior successes for this program even if at a different location.
f) Funding - Describe the different funding sources for the program. What is your long term plan for sustaining the project (if applicable)? How will you secure funding for the remainder of the budget for the current year? What is the status of other fund-raising for this program and what will happen if funds are not received to fund the entire budget for this project?
In addition to the three page Letter of Inquiry, please include:
g) Budget - You should include all sources of revenue and each category for expenses for the entire program, not just the portion for which you are seeking funding. The budget should include all expenditures for the program, including in-kind contributions.
h) Tax exempt status - Include a copy of an IRS tax exempt status letter no more than 18 months old. If you need an updated status letter, call the IRS at 1-877-829-5500 and they will fax a copy to you.
